Who we are

US Health & Wellness Network ("USHWN," "we," "our") operates Connect as a DBA, including Connect Telehealth and Connect Scripts. We provide healthcare technology, platform, coordination, and business support services to health brands, clinics, provider organizations, and other business customers.

Information we collect

• Contact information. Name, email, phone, company, and details you choose to send.
• Account and platform information. Login, organization, configuration, support, and activity information.
• Transaction information. Purchase, billing, and payment status supplied by you or an approved payment provider. We do not store full payment-card numbers on this website.
• Device and usage information. IP address, browser, device, pages viewed, referral source, and similar technical data used to run and improve the service.
• Information processed for customers. Data a business customer places in the platform is processed under that customer’s agreement and instructions.

How we use information

We use information to operate and secure our websites and platforms, create and support accounts, coordinate approved services, communicate with customers and prospective customers, process transactions, improve performance, prevent fraud or misuse, and meet legal obligations.

How information is shared

We do not sell personal information. We may share information with service providers that help run the platform; with a customer’s authorized providers, pharmacies, payment processors, or operating partners; when you direct us to; or when required to protect people, the platform, our rights, or comply with law. Each recipient receives only the information reasonably needed for its role.

Security and retention

We use administrative, technical, and physical safeguards designed to protect the information we maintain. No system is completely risk-free. We keep information only as long as reasonably needed for the service, contracts, security, dispute handling, and legal requirements, then delete or de-identify it when appropriate.

Connect Telehealth CRM software is designed to support HIPAA-compliant workflows. A BAA is available for eligible customers and must be in place before PHI is used in the platform. Each customer remains responsible for its configuration, notices, consents, workforce practices, and lawful use.

Your choices

You may ask to access, correct, or delete personal information where applicable; opt out of nonessential marketing; or ask how information is handled. Browser settings can limit cookies, though some site functions may stop working. We will verify a request before acting and may keep information when the law, a contract, security, or another permitted purpose requires it.

Health information has a separate notice

This website Privacy Policy is not the Notice of Privacy Practices. When USHWN maintains protected health information for a covered healthcare workflow, the Notice of Privacy Practices explains the applicable uses, disclosures, and individual rights. A healthcare brand, provider, pharmacy, or health plan may also give you its own notice.

The public contact and scheduling forms on connect-go.com are not patient-intake channels. Do not put diagnoses, medications, treatment details, or other health information in those general fields.

We may update this policy as our services or legal requirements change. The effective date at the top identifies the current version.
