01
This notice describes how medical information about you may be used and disclosed and how you can get access to this information.
US Health & Wellness Network ("USHWN") operates Connect as a DBA, including Connect Telehealth and Connect Scripts. This notice applies when USHWN maintains protected health information for a covered healthcare program. Your healthcare brand, independent provider, pharmacy, health plan, or other care partner may provide a separate notice that applies to its own records and services.

03
For certain uses—such as sharing information with family or friends involved in your care, fundraising, or some communications—you may tell the responsible healthcare organization what you prefer. Marketing, sale of protected health information, and most uses not described here require written authorization unless the law allows otherwise. You may revoke an authorization in writing for future use.
04
Authorized information may be used or shared for treatment, payment, and healthcare operations; to run and secure the configured platform; and to coordinate work among the healthcare brand, independent provider, pharmacy, payment processor, and other approved services.
Information may also be used or shared when the law permits or requires it—for public health and safety, health oversight, workers’ compensation, judicial or administrative matters, law enforcement, organ donation, coroners or funeral directors, certain research, or to prevent a serious threat. Any use is limited to what is reasonably needed for the purpose.
05
We protect the privacy and security of information we maintain, follow the notice currently in effect, and notify affected people when required after a breach of unsecured protected health information. We do not sell patient data or use protected health information from customer workflows for advertising.
Connect Telehealth CRM software is designed to support HIPAA-compliant workflows. A BAA is available for eligible customers and must be in place before PHI is used in the platform. Each customer remains responsible for its configuration, notices, consents, workforce practices, and lawful use.
06
Connect provides technology and operating support. Independent licensed providers deliver medical care and make clinical decisions. Independent licensed pharmacies decide whether they can accept a prescription and handle dispensing, fulfillment, shipping, and medication-related communication. Each organization is responsible for its own records and privacy duties.
The public contact and scheduling forms on connect-go.com are not patient-intake channels. Do not put diagnoses, medications, treatment details, or other health information in those general fields.
07
We may change this notice and apply the updated notice to information already maintained, as permitted by law. The effective date at the top shows the current version.
