Connect supports intake, payment, communication, provider review, and pharmacy handoff.
Terms of Use
These terms cover patient-facing pages, intake, payment, communications, and connected care workflows powered by Connect.
Effective October 16, 2026
Connect supports intake, payment, communication, provider review, and pharmacy handoff.
Independent licensed providers make every clinical and prescribing decision.
Independent licensed pharmacies control prescription acceptance, dispensing, and fulfillment.
01
These Connect Patient Terms apply when you use a patient-facing website, intake, payment page, communication channel, or care workflow powered by Connect. Connect is a DBA of US Health & Wellness Network ("USHWN," "we," "us," or "our"). By continuing, you agree to these terms, the Privacy Policy, and the program-specific terms shown by the healthcare brand serving you.
02
Connect Telehealth supports the branded patient front end, CRM, hosted intake, payments, and communications. Connect Scripts supports provider review, documentation, outcomes, and configured e-prescribing. USHWN provides technology, administrative coordination, and operating support.
Connect Telehealth and USHWN do not practice medicine or dispense medications.
03
Independent licensed providers make all clinical decisions, and a prescription is never guaranteed. A provider decides whether care or a prescription is clinically appropriate based on the information available, applicable law, and professional judgment.
Completing intake or payment does not mean treatment has been approved. If you are experiencing a medical emergency, call 911 or your local emergency service.
04
Provide complete, current, and accurate information. Do not impersonate another person, share credentials, interfere with the service, submit unlawful content, or use the workflow for an unauthorized purpose. Tell the healthcare brand or provider when health, medication, allergy, identity, location, or contact information changes.
05
By requesting remote care, you consent to the use of telehealth technology and electronic communications as described during intake. Telehealth has limits, including possible technical failures and the absence of an in-person exam. You may receive operational email, text, or phone updates when permitted by your choices and applicable law. Carrier charges may apply.
06
Pricing and the party collecting payment are shown at checkout. Payment status stays separate from the clinical decision. A provider-review fee is earned when the independent provider completes the review, regardless of the clinical outcome. The program’s written cancellation rules and our Refund Policy explain other refund eligibility.
The Connect CRM software license for business customers is $0 for the first year, then $99/month, plus utilities. That business pricing does not set a patient’s care, medication, pharmacy, or shipping charges.
07
When a provider sends a prescription, an independent licensed pharmacy decides whether it can accept and dispense it. The pharmacy handles medication pricing, compounding when applicable, fulfillment, shipping, and medication-related questions. Pharmacy acceptance, inventory, and delivery are separate from the provider’s decision.
08
Health information is handled as described in the applicable privacy notice and program agreements. Connect Telehealth CRM software is designed to support HIPAA-compliant workflows. A BAA is available for eligible business customers and must be in place before PHI is used in the platform.
Do not send health information through a general sales, email, or scheduling field. Use the secure patient workflow provided by your healthcare brand.
09
Providers, pharmacies, payment processors, communications services, prescribing networks, and other connected services are independent. Their professional decisions, services, availability, pricing, privacy practices, and terms are their own.
10
The site, software, workflow design, brand assets, and original content are owned by USHWN or its licensors. You may use them only for their intended purpose. You may not bypass security, scrape restricted content, reverse engineer protected components, or disrupt the service.
11
We work to keep the technology available and accurate, but online services can be delayed or interrupted. To the extent permitted by law, the service is provided "as is" and "as available." Nothing here limits a right or responsibility that cannot legally be limited.
12
We may update these terms as services or legal requirements change. The effective date at the top shows the current version. Program-specific terms or a separate written agreement control if they conflict with these general patient terms.
